HIPAA Policy

Hope Alive Counseling Centers (“us”, “we”, or “our”) is committed to maintaining the privacy of your protected health information (PHI) in accordance with applicable law and has established this HIPAA Policy to explain how we safeguard your privacy.

1. COLLECTION OF INFORMATION
We collect and maintain your PHI when you provide it to us, when we provide treatment, when we bill for healthcare services, and when we collect information from other health care providers or hospitals.

2. USE AND DISCLOSURE OF INFORMATION
We use and disclose your PHI for treatment, payment, healthcare operations, and when required or permitted by law. We may disclose PHI to our business associates, but we have a written contract with each of them that requires them to protect your PHI. We do not sell or rent your PHI without your explicit authorization.

3. YOUR RIGHTS
Under the HIPAA Privacy Rule, you have the following rights:
Right to Inspect and Copy:
You have the right to inspect and obtain a copy of your PHI.
Right to Amend:
If you feel that the PHI we have about you is incorrect or incomplete, you have the right to request an amendment.
Right to an Accounting of Disclosures:
You have the right to request an accounting of certain disclosures of your PHI.
Right to Request Restrictions:
You have the right to request a restriction or limitation on the use or disclosure of your PHI for treatment, payment, or health care operations.
Right to Request Confidential Communications:
You have the right to request that we communicate with you about medical matters in a certain way or at a certain location.


4. SECURITY
We use administrative, physical, and technical safeguards to ensure the confidentiality, integrity, and security of your electronic PHI. Despite this, the transmission of information via the internet is not completely secure, and we cannot guarantee the security of your PHI transmitted through our site.


5. BREACH NOTIFICATION
In the event of a breach of unsecured PHI, we will notify you as required by law. If you have provided us with a current email address, we may use email to communicate information related to the breach.


6. CHANGES TO THIS POLICY
We reserve the right to modify this HIPAA Policy and our privacy practices at any time, provided such changes are compliant with applicable law. If we make any changes, we will update this HIPAA Policy on our website.

COMPLAINTS
If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services. All complaints must be submitted in writing. You will not be penalized for filing a complaint.


This HIPAA Policy is in effect as of August 6, 2023. Your use of our services following these changes means that you accept the revised HIPAA Policy. Please read this policy carefully and keep a copy for your records.
